Parent Code of Conduct Agreement*
We, Southern Utah Nevada Youth Football League (SUNYFL), have implemented the following Parent Code of Conduct Agreement for the important message it holds about the proper role of parents in supporting their child in sports. Parents should read, understand this form prior to their child participating in our program. Any parent guilty of improper conduct at any game or practice will be asked to leave the facility and be suspended from the game. Repeat violations may cause multiple game suspensions, or the season forfeiture of the privilege of attending all games as listed below. I therefore agree: 1. I will remember that children participate to have fun and that the games are for the youth, not adults. 2. I will remember that “wins” are based on my child’s performance, teamwork, and playing within the rules. 3. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing, taunting, or using profane language or gestures. 4. I will never ridicule or yell at my child or other participants for making a mistake or losing a game. 5. I will respect officials and their authority during games and will never question, discuss, or confront officials or coaches at the field. I understand that it is impossible for any referee to catch everything that goes on during such a fast-paced game as football. Watching from the stands or the bench is not the same as watching and following the game on the field. Shouting abuse at officials does not contribute to the game, it only communicates to those around us that we need to prove to others how much we know about the game by belittling the official. During the season, everyone should expect some mistakes made by players, coaches and officials during games. 6. If I wish to calmly discuss the actions of the coach, I will wait 24-hours before calling the coach to make arrangements for a face-to-face discussion. No coach is required to listen to complaints over the phone or directly following a game or practice. 7. If I have a concern that I am unable to resolve with our coach (after observing the 24-hour rule) I will contact the appropriate person within SUNYFL. 8. I will demand a sports environment for my child that is free from drugs, tobacco, and alcohol and I will refrain from their use at all sports events. 9. I will insure that my child will attend all games and practices. When attendance is not possible, I agree to inform the coach or team manager in advance. 10. I will remember coaches are not babysitters. Parents are ultimately responsible for the actions of their child while at the field or any other team function. 11. I will control non-playing children for their own safety and comfort of others. 12. Conduct Subject to Discipline: A. Making physical contact with any player, coach, official, league representative, field personnel or spectator. B. Taunting or threatening any player, coach, official, league representative, football personnel or spectator. C. Using profane and/or vulgar language or mannerisms. D. Going onto the playing field. E. Being involved in any activity that would warrant the summoning of law enforcement officials. F. Any other conduct that is not in compliance with the bylaws of SUNYFL. I agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following: • Verbal warning by official, head coach, and/or Board Member of any District of SUNYFL. • Written warning • Parental game suspension with written documentation of incident kept on file by SUNYFL. • Parental seasonal suspension

Terms of Service*
This section to be filled out by Parent or Guardian of Athlete Please read and agree to the terms and conditions of the waiver listed below: In consideration of being allowed to participate in any way in the SUNYFL, and related events and activities, the undersigned: 1. Agree that the parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate. 2. Understand and acknowledge that as a result of participation, an injury could occur that may require medical attention, and that on occasion the coaches and staff of SUNYFL, local area staff, and available medical personnel may unable to contact me immediately for my consent for emergency medical care. I do hereby authorize the staff of SUNYFL to consent on my behalf as they may deem necessary to such emergency medical care, including ambulance transport and hospital care, as may be deemed necessary under the then existing circumstances by available medical personnel, including ordering injections and /or anesthesia and/or surgery. In so doing, I assume responsibility for the expenses of any such care not covered by my insurance. 3. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time. 4. Assume all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent total disability or death. 5. Release, waive, discharge and covenant not to sue SUNYFL, their respective Administrators, directors, agents, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as “releases” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise. 6. I confirm my understanding and consent that by participating in SUNYFL Football Camp, my child/ward may be photographed, identified and/or interviewed by people providing information for school publications or the media. I give my permission for SUNYFL to publish, on its website or in school publications, photographs and other information which may identify my child/ward related to my child’s participation in SUNYFL Football Camp. ** Refund Policy: We have NO REFUND policy. SUNYFL enforces a strict no-refund and no-transfer policy, with no exceptions
